How Steel City Medical collects, uses and protects your personal health information, including what you send us through our new-patient waitlist form.
Who is responsible for your information
Our physicians belong to Ottawa-Sherman Medical Associates FHO, a Family Health Organization. Under Ontario's Personal Health Information Protection Act, 2004 (PHIPA), the FHO is the health information custodian for the information we hold about you. Our staff, and the Hamilton Family Health Team, act as its agents and follow the Hamilton Family Health Team's privacy policy, which applies to our patients' health records.
The new-patient waitlist form
The waitlist form asks for your name, contact details, health card number, date of birth and a short medical history. We use it only to manage the waitlist, to match you with a provider and to contact you when a spot opens.
- Who can see it. Only the clinic staff who manage the waitlist. The form is hosted in Google Forms, in an account the clinic controls.
- How long we keep it. If you become our patient, your submission becomes part of your health record. Otherwise, we keep it only while you are on the waitlist.
- Leaving the waitlist. Ask at any time and we will take you off the waitlist right away and permanently delete your submission within 30 days.
